Dr Anton van Dellen was instructed to represent the family in a fresh inquest after the previous inquest was quashed by consent.
The Pre-Inquest Review hearing was heard remotely before the Senior Coroner for Lancashire and Blackburn with Darwen, Dr James Adeley. The Senior Coroner added OFSTED as another Interested Person under section 47(2)(m) of the Coroners and Justice Act 2009.
